Casting Survey -Canada 2015

Thank you for your interest in participating in the 2015 Titleist Golf Ball TV Commercial Shoot. Please complete the following form in its entirety including the upload a photograph and a short video with the questions answered. Completed form must be submitted by Wednesday, September 16.

1. The shoot will take place at Cabot Cliffs on Tuesday, September 22nd or Wednesday, September 23rd and will be an all-day event.  You will only be asked to participate in one shoot.



Please note that if selected, you will be asked to attend a 1 hr casting call on an evening the week prior to the shoot.
